Voluntary Plans

All voluntary plans require payroll deductions and will continue year-to-year unless you cancel them.  The only exception is for American Fidelity 125 medical and childcare reimbursement.  It is the employees responsibility to keep their voluntary plans informed of name, address and salary changes.  Click on the links below for more information.
